KKK Costume.
I don't need to explain why this is a bad idea. Also, when you dress your kid up as a ghost, make sure the hat part isn't too pointy. And if you don't find the KKK idea offensive, perhaps you already have a costume in your closet...
Hitler Costume.
If you Google "Hitler Halloween Costume" you will find a number of people enquiring such things as:
"Is it wrong to dress up as Hitler on Halloween?"
Uh....YES.
"How do I make a Hitler costume for Halloween?"
Well, let me see, you take 1 part ignorance, 1 part stupidity, 3 parts evil dictator and 3 parts intolerance. Mix well...
Or in fact, any kind of Nazi uniform. Prince Harry found this out when he decided to go Nazi for a costume party, expecting that nobody would notice, take a photo or indeed take offense.
This probably applies to Stalin, Mugabe, Pol Pot and various other vicious dictator costumes, just to be on the safe side...
World Trade Centre Costume.
These WTC costumes should also be called WTF. It's just wrong. If you want to offend pretty much everyone in the US and allied countries, go right ahead...
These costumes are incredibly scary, just not in the way that characterizes the Halloween holiday season. Stay safe, kids.
